In this paper the author describes, from the perspective of a recently retired Director of Rail Investigations, how Canadian railway safety management changed over the previous twelve years. Issues covered include changes to the regulatory, industry and investigative environment and use examples of accidents investigated to highlight problems encountered during the author’s time working at the Transportation Safety Board of Canada. The issues presented are used to draw conclusions, with lessons learned, which can perhaps have some application in other countries.
