This paper reviews three serious US accidents between 2002 – 2009 involving freight trains, passenger trains and a metropolitan subway. In all three cases investigators found that the use of cell phones or text messaging contributed to the accidents. Next the paper examines the policies and practices as well as the challenges of ensuring compliance.
