Rail safety regulators and other official bodies will tell you that, as a rail organisation, it is your duty to try to learn from the mistakes and incidents of others, not just in the rail industry and not just in your country but in international rail incidents and from incidents occurring in other industry sectors. this paper will show a simple way to quickly monitor and follow up major incidents from elsewhere and explain how the fundamentals of an incident are often universal.
